After signing the contract, you are allowed to use healthcare services founded by NFZ:

• you can see a doctor for a medical appointment
• you can receive hospital treatment
• you can use reimbursements of medications and medical devices

Places where you can go to receive services are labelled with NFZ logo.

Documents required to sign the contract:

• application for voluntary health insurance (form available in the NFZ office and on the website)
• insurance status declaration (form available in the NFZ office and on the website) 
• passport
• certificate of assigning the PESEL number
• certificate confirming the student status
• residence document, e.g. visa, TRC
